Reliable Drug Testing Solutions

 Pro-Lab Drug Testing Services LLC is committed to providing accurate, reliable, and confidential testing services. Please review the following policies prior to your visit. 

 Appointments are encouraged and prioritized; however, walk-ins are welcome during normal business hours and are subject to availability. Certain services may require scheduling in advance. 

A deposit is required when scheduling the following services:

• Hair Follicle Drug Testing 
• Nail Drug Testing 
• Legal DNA Testing
   

Deposits apply toward the total cost of service. Deposits may be waived when services are scheduled and billed through an employer or Third-Party Administrator (TPA).

 All deposits and payments are non-refundable once an appointment is scheduled or the collection process has begun. Missed appointments, late arrivals, or refusals to test are non-refundable. 

 Valid, government-issued photo identification is required for all testing services. Failure to present proper ID may result in rescheduling or cancellation. 

 Late arrivals may experience wait times or be required to reschedule based on availability. No-shows may result in forfeiture of the deposit or payment. 

 Results are released only to authorized parties and are typically delivered securely. Turnaround times begin once the laboratory receives the specimen. Recollection may be required in certain cases and additional fees may apply unless covered by an employer or TPA. 

 Testing scheduled through an employer or TPA is billed directly to that entity. Individuals scheduling services independently are responsible for payment at the time of service.
